I think the oncologist is trying to explain the decision to do a mastectomy, rather than just a wide local excision (which leaves most of the breast behind) + radiation, which is the norm for older women if they have small tumours. She'll get a better cosmetic outcome by having the whole breast removed, and being reconstructed at a later date once her other breast has fully developed, so they can match them. I hope that made sense - my brain is a bit slow at the moment. But, don't worry, they would have recommended radiation if she would have benefited from it (some people do still need it even after a mastectomy, depending on certain factors). What a brave girl!
